We are currently seeking a dedicated and experienced Certified Diabetes Educator (CDE) to join our team.
This role is crucial in empowering patients to effectively manage their diabetes through education, support, and personalized care.
Responsibilities: * Develop and conduct training with our Monitoring Specialists.
* Review current onboarding processes for new patients and make modifications as needed.
* Conduct individualized assessments of patients with diabetes to determine their educational needs and develop personalized care plans.
* Provide clear and accurate information to patients about diabetes, its management, and lifestyle modifications through educational materials and group education programs.
* Collaborate with healthcare professionals, including physicians, nurses, pharmacists, and dietitians, to ensure integrated and comprehensive diabetes care.
* Stay informed about the latest advancements, research, and best practices in diabetes care and management.
* Utilize a patient-centered approach to encourage and support individuals in making positive changes to their lifestyle, diet, and medication adherence.
* Monitor and evaluate the progress of patients in achieving their diabetes management goals.
* Maintain accurate and up-to-date records of patient interactions and educational sessions.
* Utilize educational materials, technology, and other resources effectively to enhance patient understanding and engagement.
* Stay abreast of changes in healthcare policies and regulations related to diabetes education and counseling.
Qualifications: * Current certification as a Diabetes Educator (CDE) is required.
* Licensed RN, Dietitian, or a related field.
* Minimum of 2 years of experience in providing diabetes education and counseling.
* Fluent in English and Spanish preferred.
* Excellent interpersonal and communication skills.
* Ability to work collaboratively in a multidisciplinary healthcare team.
* Strong organizational and documentation skills.
* Cultural competence and ability to work with diverse populations.
* Proficiency in using educational materials and technology for patient education.
* Proficiency in diabetic-related medical technologies and software.
